Communication is critical to the success of a hospitalist program. HMR assists Hospitalist physician groups in:

• Communicating regularly with the patient’s primary care physician
• Coordinating discharge planning and follow-up care in cooperation with the patient care coordinator
• Leading all necessary coordination with the hospital care team
• Actively communicating between provider and patient
• Timely communication with the family
